Primary Care – I start by establishing a strong clinical foundation. Whether you need a routine physical, chronic disease management, or preventive screenings, I focus on your long-term healthspan.

Hormone therapy - I start with a comprehensive assessment and detailed lab testing to evaluate your hormone levels and overall health. If we move forward with treatment, you’ll receive ongoing support and customized care through both virtual and in-person visits.

Wellness management - This is the path for deep-diving into data that insurance usually doesn’t cover - like body composition and wearable insights. It’s designed for those who want a high-level partnership and accountability in fine-tuning their health and wellness goals. This is an optional add-on service for my primary care patients or a cash-pay option for those not using insurance.

Dr. Miguel Rosa, DNP, APRN, FNP-BC

Miguel’s journey into founding his own health and wellness practice was born from both personal evolution and professional insight. With a long history in endurance sports — including completing eight marathons and cycling Haleakala, the world’s longest paved climb — Miguel has developed a deep understanding of training, recovery, and the physical shifts that come with aging. His experience allows him to approach wellness not only from a clinical standpoint but from the real-world perspective.

As he neared his 40s, Miguel began to notice subtle but undeniable changes in his performance, recovery, and overall energy. That self-awareness sparked a deep dive into functional medicine and hormone optimization, which quickly evolved from personal curiosity into a professional calling.

As a nurse practitioner and someone who truly lives what he teaches, Miguel brings both clinical expertise and personal experience to his work. On his spare time he leads indoor cycling classes and stays deeply connected to movement — not just for fitness, but for focus, joy, and clarity.

With a natural passion for concierge-style care, Miguel wanted to create a space where people can have easy access to their provider, feel heard, and explore better ways to care for their health — without the pressure and burnout that can come from today’s typical healthcare model.

His goal is to build a small, intentional practice that grows organically — where trust and personalized support are truly the norm.

Do you accept insurance?

I am currently transitioning my practice to a hybrid model. While I am private-pay today, I expect to be in-network with multiple insurance carriers by Summer 2026. If you'd like to work with me using your insurance, please contact me so I can add you to my priority notification list.


Can see you if I have Medicaid or Medicare?

Due to federal and state regulations, I cannot accept out-of-pocket (cash) payments from Medicare or Medicaid members for covered medical services. While I cannot see you for primary care today, I am happy to add you to my priority list now so you are the first to be notified when my in-network status is active this summer.


What is the difference between Primary Care and Wellness Management?

Primary Care covers your essential medical needs, from routine physicals to chronic disease management.

Wellness Management is a separate, specialized path for those who want to deep-dive into longevity data insurance doesn't cover—like InBody scans, metabolic health, and wearable device integration.


Do I need a membership to see you for Primary Care?

No. Once I am in-network with your insurance, your primary care visits are billed through your plan (subject to your standard co-pays). My monthly membership options are strictly for cash-pay patients or those enrolled in the Wellness Management program for non-covered services.
