A Note from Miguel: What actually happens in the first 12 months of TRT

Slow steady improvement is what we're looking for here.

Most people starting testosterone replacement therapy expect to feel different within the first few weeks. If no dramatic change is noted, the instinct if to either increase the dose or question whether it's working at all.

I get it...

We live in an instant gratification culture.

But TRT doesn't work that way...

Here's what the timeline looks like:

Weeks 3-6: Libido and sexual interest tends to be the first symptoms to improve. Most people notice a shift around week 3, I call this the honeymoon phase because it plateaus around week 6. Energy, mood, and overall sense of well being may start to shift.

Months 3-6: This is when body composition starts to change. Some notice reductions in fat mass and gains in lean mass. This typically occurs around week 12-16 and continues through month 6 and beyond. Erectile function improves in males, and females may experience increased sexual function/desire. Lipid markers start to shift around week 4, but we don't see maximum benefit for 6 months to a year.

Months 6-12: Bone mineral density improvments become detectable around 6 months and can continue for years. Lipid and metabolic benefits reach their maximum. This is when the physical and mental shifts start feeling like a new normal.

The focus then shifts from fixing a baseline deficiency to true lifestyle optimization. Fine tuning nutrition, training/activity, and health habits.

I've had a few established patients for almost a year now (it's crazy to think August is right around the corner). Over the course of this year we've adjusted doses, started supplements, monitored lab work, adjusted treatment plans, and little by little we've noticed little shifts.

Mood, body composition, hot flashes, sexual function/desire, fatigue. I'm happy to report that we've seen a positive trend, and it didn't happen overnight.

I like to quantify hormonal shifts by sending out periodic symptom questionnaires which allows patients to provide raw honest responses to some sensitive questions that may have been answered otherwise during an office visit. This lets me evaluate progression over time, and so far, this has worked really well.
